These Terms of Use ("Terms") govern your access to and use of Tabula Medica applications and services ("Services"), operated by Tabula Medica LLC ("we," "us," or "our"). By using our Services you agree to these Terms.
Tabula Medica provides personal health record management tools that help you gather, organize, and share your own health information. The Services are for personal, non-commercial use unless you have a separate written agreement with us.
The Services are for informational and organizational purposes only. Nothing in the Services constitutes medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare provider with questions about a medical condition.
You must be at least 18 years old to create an account. By using the Services you represent that you meet this requirement and that the information you provide is accurate.
You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. Notify us immediately at support@tabulamedica.com if you believe your account has been compromised.
Certain features are available via optional auto-renewable subscriptions purchased through the App Store or Google Play. Subscription fees are billed to your store account at confirmation of purchase and automatically renew unless cancelled at least 24 hours before the end of the current period. You can manage and cancel subscriptions in your store account settings at any time. We do not process subscription payments directly; all billing is handled by Apple or Google under their respective terms.
You agree not to: (a) use the Services for any unlawful purpose; (b) attempt to gain unauthorized access to any system or network; (c) transmit malicious code; (d) scrape or harvest data without our written consent; or (e) resell or sublicense access to the Services.
Your use of the Services is also governed by our Privacy Policy. We take the security of your health information seriously and comply with applicable law including HIPAA where applicable.
The Services and their content (excluding your health records) are owned by Tabula Medica LLC and protected by applicable intellectual property laws. You retain ownership of your health data.
THE SERVICES ARE PROVIDED "AS IS" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, TABULA MEDICA LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES ARISING FROM YOUR USE OF THE SERVICES.
These Terms are governed by the laws of the Commonwealth of Virginia, without regard to conflict of law principles.
We may update these Terms and will post the revised version with a new effective date. Continued use of the Services after changes constitutes acceptance of the updated Terms.
