Tabula Cognita is a cognitive assessment platform for licensed clinicians. This policy explains how we handle information, including protected health information (PHI) that clinicians enter about their patients.
We use information solely to provide the clinical assessment service to the clinician and their practice: to run assessments, generate documentation, and maintain security and audit records. We do not sell information and do not use PHI for advertising.
Where Tabula Medica LLC acts as a business associate, PHI is handled in accordance with applicable Business Associate Agreements and HIPAA. We share information only with service providers (such as secure cloud infrastructure) under agreements that require appropriate safeguards, and as required by law.
We use encryption in transit and at rest, access controls, audit logging, and other administrative, physical, and technical safeguards designed to protect PHI.
Assessment records are retained per the practice's requirements and applicable law. Patients should direct requests about their records to their clinician/covered entity; clinicians may contact us for account and data requests.
Tabula Cognita is a professional tool used by clinicians and is not directed to children as end users.
Tabula Cognita supports qualified healthcare professionals and does not provide a diagnosis. It is not a substitute for clinical judgment.
