Tabula Cognita — Privacy Policy

Tabula Cognita is a cognitive assessment platform for licensed clinicians. This policy explains how we handle information, including protected health information (PHI) that clinicians enter about their patients.

Information we process

• Clinician account information — name, email, professional credentials used to verify and manage access.
• Assessment data (PHI) — patient information and cognitive assessment results that a clinician enters to conduct and document evaluations.
• Usage and device information — logs and diagnostics used to operate and secure the service.

How we use information

We use information solely to provide the clinical assessment service to the clinician and their practice: to run assessments, generate documentation, and maintain security and audit records. We do not sell information and do not use PHI for advertising.

HIPAA and how information is shared

Where Tabula Medica LLC acts as a business associate, PHI is handled in accordance with applicable Business Associate Agreements and HIPAA. We share information only with service providers (such as secure cloud infrastructure) under agreements that require appropriate safeguards, and as required by law.

Security

We use encryption in transit and at rest, access controls, audit logging, and other administrative, physical, and technical safeguards designed to protect PHI.

Retention and your rights

Assessment records are retained per the practice's requirements and applicable law. Patients should direct requests about their records to their clinician/covered entity; clinicians may contact us for account and data requests.

Children

Tabula Cognita is a professional tool used by clinicians and is not directed to children as end users.

Clinical note

Tabula Cognita supports qualified healthcare professionals and does not provide a diagnosis. It is not a substitute for clinical judgment.
